We are an out-of-network provider and can issue superbills upon request. These can be submitted to your insurance company for possible reimbursement if you have a PPO plan. We recommend contacting your insurance provider directly to ask about your out-of-network mental health benefits and reimbursement eligibility.

Forms will be filled out only for established patients. Forms that may be provided, at the clinician’s discretion, could be accomodatin, ESA letters, etc We do not offer long term disability certification. Short-term disability may be considered for established patients who are in a psychiatric day program that requires time off work. This will be at the clinician’s discretion. Any form requiring evaluation and more than 20 minutes of the clinicians time will be charged at a rate of $150
